Job summary

The Commonwealth of Pennsylvania is seeking County Caseworkers to build rapport and professional relationships with consumers of all ages in services and their families. Caseworkers advocate for the consumer, monitor the delivery of services, link individuals to services, develop social supports, and encourage self-determination.

Full-time positions include healthcare coverage, pension plan, student loan reimbursement program, flexible spending account, and paid holidays.

Qualifications

• Six months of experience as a County Caseworker 1; or
• Successful completion of the County Social Casework Intern program; or
• A bachelor's degree with a social welfare major; or
• A bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of professional social casework experience in a public or private social service agency; or
• Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.

Responsibilities

• Monitor, link, and evaluate services. Link to other services and make referrals as needed
• Provide regular direct individual contact and face-to-face case management while meeting productivity standards
• Schedule and facilitate meetings to develop an individualized service plan with involvement of the individual and/or family
• Enter necessary information and maintain information in computerized databases, state computer systems, and in charts in accordance with regulations and agency policy
• Offer crisis intervention services and coordinate assistance as needed
• Work with Agency staff, providers, and community resources to support individuals
• Link individuals to transportation services or transport individuals to appointments as needed
