Job summary

The Commonwealth of Pennsylvania, Bedford-Somerset DBHS, is seeking County Caseworkers to build rapport with consumers and their families, advocate for services, monitor delivery, and support self-determination. This role focuses on connecting people with essential resources and tracking progress in developmental delays, mental health, or intellectual disabilities.

Qualifications

• Bachelor's degree with 12 credits in sociology, social welfare, psychology, gerontology or related social sciences.
• Two years of experience as County Social Services Aide 3 or equivalent paraprofessional case management.
• Educational requirements can be met within two months of graduating.
• Equivalent combination of education and experience acceptable.

Responsibilities

• Service Monitoring: monitor, link, and evaluate services and referrals.
• Direct Contact: provide regular face-to-face case management and meet productivity standards.
• Plan Development: facilitate meetings to create individualized service plans with client/family.
• Information Management: enter data into state systems and charts per regulations.
• Crisis Support: provide crisis intervention and coordinate assistance.
• Collaboration: work with staff, providers, and community resources.
• Transportation Support: link to transportation or transport as needed.

Education

Bachelor's degree with 12 credits in sociology/social sciences

Two years experience as County Social Services Aide 3

Two years college coursework including 12 credits in sociology/social sciences

Equivalent combination of experience and training with 12 sociology/social science credits and one year experience
