Job summary

The Commonwealth of Pennsylvania is seeking a County Caseworker 2 in Danville, PA. You will support individuals and families by guiding access to services, building relationships, and offering steady community support. This role emphasizes compassion and teamwork within a county MH/ID unit.

Responsibilities include case management, resource coordination, and home visits to assess and assist with needs. Strong communication with clients and partners is essential.

Qualifications

• Six months of experience as a County Caseworker 1; or completion of the County Social Casework Intern program; or a bachelor's degree with a social welfare major; or a bachelor's with 12 credits in sociology, social welfare, psychology, gerontology, criminal justice or related fields and one year of professional social casework in a public/private agency; or any equivalent combination including 12 credits in the related fields.

Responsibilities

• Case Management: Coordinate services for consumers and families with developmental disabilities or serious mental illness.
• Resource Coordination: Locate community supports and guide access to services.
• Home Visits: Meet in homes and communities to assess needs and provide ongoing support.
• Documentation Review: Examine information to ensure accurate planning and coordination of services.
• Communication Support: Maintain communication with consumers, families, providers and agencies.
