Job summary

Medix in Texas seeks a Connected Care Assistant dedicated to world-class member service. Working with the clinical team, you will coordinate care for members with complex or chronic conditions and support providers and caregivers.

You will handle member calls and chats, outreach, and navigate health IT systems while maintaining HIPAA confidentiality. The role emphasizes compassionate communication, multitasking, and the ability to learn new platforms quickly.

Qualifications

• Experience in case management or healthcare setting preferred.
• Proficiency in motivational interviewing techniques.
• Ability to multitask and remain flexible in a changing environment.
• Strong telephone skills and effective communication, both verbal and written.
• Maintains patient confidentiality per HIPAA requirements.

Responsibilities

• Make member calls, chats, and referrals into CCP.
• Engage at-risk members through targeted outreach.
• Use autodialer tooling for outreach to eligible members.
• Manage and triage care plan task queues.
• Coordinate care for members and families as directed by the clinical team.
• Recommend available benefits through employer benefits navigation.
• Initiate cases such as Provider Match or Claims as directed.
• Compose letters and mailings to members and providers.
• Obtain medical releases and records and triage related requests.
• Monitor email referrals and portal communications; manage portals like Ovia and ACM.
