Qualifications

• Active Medical Assistant certification or LPN license in the U.S.
• 1+ year of experience working in a clinical, community health, or primary care setting.
• Experience with EHR and patient engagement platforms.
• Familiarity with care coordination, chronic care management, or remote monitoring.

Responsibilities

• Conduct patient outreach, intake, and enrollment into care management services.
• Non-clinical or semi-clinical check-ins to gather health updates and reinforce care plans.
• Support supervising RN with tracking patient progress and scheduling follow-ups.
• Document patient interactions in the care management platform and/or EHR for billing and compliance.
• Coordinate translation services when needed to support language access.
• Follow scripted workflows and participate in team huddles and trainings.
