Job summary

Responsibilities

• Manage overall member service plan within applicable state law and contract.
• Conduct telephonic or in-person history and needs assessments using predefined questions.
• Identify members with potential clinical needs and coordinate with care team.
• Coordinate non-clinical needs for members with chronic illnesses to optimize utilization.
• Establish short and long-term goals with members, caregivers, and physicians.
• Identify members who could benefit from expanded services.
