Job summary

Concord Hospital is seeking a Patient Care Coordinator to provide front-line support for patient registration, referrals, scheduling and communications. You will be the primary access point for patients and families, delivering timely, compassionate customer service while coordinating care across departments.

The role requires a high school diploma or GED, strong customer service skills, familiarity with medical terminology, and knowledge of medical office operations including EMR and billing

Qualifications

• High School diploma or GED required; must pass competency exam.
• Demonstrated customer service ability with public, staff and patients preferred.
• Familiarity with medical terminology preferred.

Responsibilities

• Greets patients and secures signatures for all required documentation.
• Verifies insurance and demographic information; makes collection attempts and education on resources.
• Facilitates patient care between primary, specialty, and ancillary services.
• Welcomes and coordinates intake of patient information; creates and maintains medical records.
• Processes and routes all incoming calls or requests; responds to inquiries.
• Schedules, confirms or reschedules patient appointments; promotes Patient Connect enrollment.
• Distributes mail, faxes and electronic desktops; educates patients on form completion.
