Job summary

VIA Health Partners is seeking a Nurse Practitioner to provide comprehensive end-of-life care in Rutherfordton, NC. The role focuses on symptom management, patient and family counseling, and collaboration with the Medical Director and care team.

The ideal candidate holds a Graduate Degree with at least 1 year in direct patient care for life-limiting illnesses and has experience in hospice/palliative settings. On-site work with travel as needed and strong commitment to compassionate care.

Qualifications

• Minimum 1 year related experience in direct patient care with life-limiting illnesses.
• Experience in hospice and palliative care is preferred.

Responsibilities

• Provide comprehensive medical care to VIA patients.
• Diagnose, treat and manage pain and symptoms at end of life.
• Counsel patients and families on disease progression, prognosis and care options.
• Prescribe appropriate therapies and coordinate with Medical Director as needed.
• Submit billing and documentation according to policy.
