VIA Health Partners is a leading hospice and palliative care provider delivering comprehensive end-of-life services in the Charlotte area. We seek an experienced Nurse Practitioner to join our Homecare East Charlotte team, focusing on high-quality patient care and family support within a compassionate, mission-driven environment.

You will manage clinical duties, collaborate with physicians, and guide treatment plans for patients with life-limiting illnesses, while promoting comfort, dignity, and

Formación

• Master's degree in nursing required.
• Current, active and unrestricted NC RN license; official NP certification from NC Board of Nursing.
• Current and unrestricted DEA and Basic Life Support required; computer proficiency.
• Minimum 1 year related experience in hospice/palliative or related fields.

Responsabilidades

• Provides comprehensive medical care to HPCCR patients.
• Serves as medical resource for HPCCR staff and patients.
• Prescribes systemic and topical therapies including narcotics as appropriate.
• Conducts patient/family meetings to discuss goals of care and prognosis.
• Promotes high quality end-of-life care and benefits of advanced directives.
• Participates in team discussions to optimize care plans and outcomes.
