Job summary

Affinity Hospice Management LLC seeks a Nurse Practitioner to deliver comprehensive hospice care under a collaborating physician. The NP will evaluate medical history, perform physical assessments, diagnose conditions, and direct the plan of care in patients' homes and communities.

Responsibilities include coordinating with the hospice interdisciplinary team, completing face-to-face eligibility assessments, maintaining documentation, educating families, and ensuring comfort and symptom

Qualifications

• Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
• Current NP license in the state and CPR certification.
• DEA license if required by state regulations.
• Minimum 1 year NP experience with hospice or palliative care.
• Certification in CHPN or related specialty preferred.

Responsibilities

• Provide medical care and manage terminal illness under physician supervision.
• Perform history and physical exams and interpret tests.
• Develop and implement a plan of care and prescribe treatments.
• Complete hospice face-to-face visits to assess eligibility.
• Coordinate with hospice interdisciplinary team and physicians.
• Educate patients and families about end-of-life care.
• Maintain accurate clinical documentation and records.
