Job summary

Affinity Hospice Management LLC is seeking a Nurse Practitioner to deliver comprehensive hospice care under physician supervision. You will evaluate health history, develop a plan of care, and coordinate services with patients and families in home settings.

Responsibilities include conducting histories and exams, prescribing appropriate treatments, and ensuring eligibility via hospice processes in collaboration with the interdisciplinary team.

Qualifications

• Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
• Current DEA license, state nursing licensure and CPR certification.
• Master's degree with a minimum of one year Nurse Practitioner experience; hospice/palliative care experience preferred.

Responsibilities

• Provide medical care and manage terminal illness under physician supervision.
• Complete history and physical exams and document findings in the medical record.
• Develop and manage an individualized plan of care; prescribe medications and coordinate hospice services.
• Perform hospice face-to-face visits to assess eligibility in consultation with the hospice team.
• Educate patients and families; maintain accurate clinical documentation and ensure safety.
