Job summary

VIA Health Partners is a leading nonprofit hospice and palliative care provider delivering comprehensive patient- and family-centered services. We seek an experienced RN to coordinate and deliver high-quality end-of-life care in our community-based setting.

The role requires an RN license, CPR certification, and experience in inpatient or hospice settings, with CHPN preferred. We emphasize compassionate, team-based care and ongoing professional development in a supportive, non-profit environment.

Qualifications

• Diploma or associate degree in nursing is required; Bachelor’s Degree preferred.
• Active RN license is required; NLC allows multi-state license in home state.
• Current CPR certification is required; CHPN preferred.
• Computer proficiency is required.

Responsibilities

• Delivery of Patient Care: coordinate and deliver patient/caregiver care.
• Conduct nursing assessments and develop plans of care.
• Provide education to patients and families; ensure continuity of care.
• Maintain infection control and document meticulously in records.
• Communicate with IDG/patients/caregivers and collaborators.

Education

Diploma or Associate Degree in Nursing

Bachelor’s Degree preferred

Active RN license (Nursing Licensure Compact)
