Job summary

Qualifications
- Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
- Current DEA license and state nursing licensure; CPR certification.
- Master's degree with NP specialization and hospice/palliative care experience.
Responsibilities
- Provides medical care and manages terminal illness under physician supervision.
- Performs history and physical examinations and documents findings.
- Develops individualized plan of care, prescribes medications and coordinates hospice care.
- Completes hospice face to face visits to assess eligibility with interdisciplinary team.
