Job summary

Affinity Health Management is seeking a compassionate hospice aide to support patients under RN supervision. You will perform personal care, monitor condition, document observations, and assist with basic daily activities in accordance with the patient’s plan of care.

Responsibilities include safety-focused care, patient-family communication, and reporting changes to the Case Manager. Qualifications include CNA credentials, CPR, and a willingness to follow COVID guidelines.

Qualifications

• Completion of state-approved CNA program or equivalent training.
• Current Healthcare Provider CPR certification (hands-on verification).
• Covid vaccination per facility policy.
• Ability to read and follow written instructions and document care.

Responsibilities

• Provide personal care and respond promptly to patient requests.
• Communicate with patient and family respectfully and confidentially.
• Follow safety practices and use equipment properly.
• Assist with baths, grooming, oral care, and linen changes.
• Help with dressing, undressing, and feeding as assigned.
• Avoid giving homemade foods to patients.
• Monitor skin condition and prevent tissue breakdown.
• Report pain or changes to the RN Case Manager.
• Assist with ambulation and positioning as directed.
• Support self-administration reminders for medications.
• Assist with bedpans, urinals, and light homemaker tasks.
• Document care and observations accurately in EMR.
• Provide respite support to patient and family when on-site.
• Keep living area clean and orderly.
• Follow care procedures and professional conduct standards.
• Participate in QA and improvement activities.
• Respect patient and family environment and privacy.
• Adhere to reporting requirements for safety and abuse.
