Job summary

Affinity Health Management is seeking a hospice aide to support patients under RN supervision, delivering personal care and comfort per each plan of care. The role emphasizes observation, documentation, and compassionate interaction with patients and families.

The ideal candidate completes a state-approved CNA program, holds CPR certification, and is vaccinated for Covid-19. Prior hospice or long-term care experience is preferred; strong EMR documentation skills are required.

Qualifications

• State-approved CNA course or equivalent training is required.
• CPR certification (Healthcare Provider) is required, hands-on verified.
• Must be covid vaccinated per facility guidelines.
• High school diploma or GED is required.
• Ability to read and follow written instructions and document care.
• Hospice philosophy understanding and comfort with terminally ill patients.
• Prior hospice, orderly, acute, rehabilitative, or long-term care experience preferred.
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle in good working order.

Responsibilities

• Provide personal care and attend to patient requests promptly.
• Communicate with patients and families with confidentiality and ethics in mind.
• Ensure patient safety and use equipment properly.
• Assist with baths, grooming, oral hygiene, and linen changes.
• Help patient dressing and undressing as assigned.
• Assist with feeding and avoid homemade foods prepared by caregiver.
• Monitor and report skin condition to prevent tissue breakdown.
• Report pain or changes in patient condition to RN Case Manager.
• Assist with ambulation and range of motion as directed by nurse/therapist.
• Perform transfers/positioning and ROM as ordered.
• Assist with medications reminders within state regulations.
• Assist with bedpan/urinal use and light housekeeping tasks.
• Perform simple procedures with competency as ordered.
• Provide respite for patient/family when on-site, as appropriate.
• Keep living area clean and orderly.
• Adhere to care procedures and professional conduct; participate in QA activities.
• Show respect for patient and family environment and needs.
