Job summary

The Care Team Hospice is seeking a compassionate Medical Social Worker (MSW) to provide emotional support, counseling, advocacy, and resource coordination for patients and families navigating the hospice journey.

This part-time role has the potential to become full-time as patient census grows. Ohio LSW/LISW license required; MSW preferred. On-site role with a strong interdisciplinary team support and comprehensive benefits.

Qualifications

• Must hold Ohio LSW or LISW license.
• MSW from an accredited program preferred.
• Experience in hospice, home health, healthcare, or case management preferred.

Responsibilities

• Conduct comprehensive psychosocial assessments and develop individualized plans of care.
• Provide emotional support, counseling, crisis intervention, and guidance to patients, families, and caregivers.
• Assist with advance care planning, connect to community resources, and bereavement support.
• Collaborate with physicians, nurses, chaplains, and the interdisciplinary team for coordinated care.
• Maintain accurate documentation in the Homecare Homebase EMR.
