Job summary

Qualifications

• MSW from an accredited school.
• Minimum of one year experience in health care; hospice experience preferred.
• Experience in hospice care preferred.
• Strong verbal and written communication skills.
• Criminal background checks required by regulation.
• Must be a licensed driver with insured vehicle.
• Possesses CPR certification.

Responsibilities

• Assess the psychosocial status of patients and families/caregivers related to terminal illness and communicate findings to the team.
• Conduct social evaluations, plan interventions, and provide crisis intervention when necessary.
• Educate interdisciplinary team on cultural needs and advance directives.
• Develop individualized care plans with patient and family involvement.
