Job summary

Qualifications

• Requires MSW from a CSWE-accredited program.
• Minimum 1 year healthcare experience; hospice preferred.
• Experience in hospice care preferred.
• Excellent verbal and written communication and strong organizational skills.
• Background checks by regulation and driving license required; CPR certification required.

Responsibilities

• Assess psychosocial status of patients and families and communicate findings to the care team.
• Conduct social evaluations and plan interventions with consideration of high-risk situations.
• Educate interdisciplinary group on cultural considerations and advance directives.
• Assist in developing individualized plans of care and participate in interdisciplinary meetings.
• Provide crisis intervention and assist in decision making related to health care planning.
• Coordinate information and referrals with community resources and agencies.
• Support continuity of care and adapt psychosocial interventions as patient needs evolve.
