Job summary

Three Oaks Hospice, Inc. seeks an experienced Registered Nurse to administer skilled nursing care for hospice patients in their homes, coordinate care with the interdisciplinary team, patients, and families, and manage overall care coordination.

Requirements include an accredited nursing degree, current RN license, valid driver’s license, CPR, and at least 2 years of RN experience (hospice/home health preferred). On-call coverage is part of the role.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and valid driver’s license.
• Two years’ RN experience in a clinical care setting; hospice/home health preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.

Responsibilities

• Assess the patient’s physical, functional, psychosocial, and environmental needs and ADLs.
• Implement and revise the plan of care (POC) with the IDT as needed.
• Educate patient and family on disease process, self-care, end-of-life care, nutrition, and care coordination.
• Initiate preventive and rehabilitative nursing procedures and document progress.
• Coordinate all patient/family services with the IDT and referring agency.
• Provide on-call nursing coverage as assigned.
