Three Oaks Hospice, Inc. is seeking compassionate registered nurses to provide skilled nursing care to patients in their homes. You will coordinate with the inter-disciplinary team and families to deliver high-quality hospice care.

The role emphasizes assessment, care planning, patient education, and on-call coverage. Candidates should have an Associate or Bachelor of Science in Nursing, an active RN license, and CPR certification.

Qualificações

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN.
• Two years’ RN experience in clinical care; hospice/home health preferred.
• Current CPR certification.
• Proficient with computer use; MS Office preferred.
• Strong interpersonal, communication, and organizational skills.
• Able to manage caregiver and family emotional needs.

Responsabilidades

• Initial and ongoing assessments of the terminal disease impact on patient needs.
• Implement the individualized POC and revise as necessary.
• Coordinate care with the IDT and family; educate patient/family.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare progress notes showing progress toward goals.
• Coordinate all patient/family services with the IDT.
• Inform physician of changes in patient needs and outcomes.
• Determine scope and frequency of services based on acuity.
• Provide on-call coverage as assigned.
