Job summary

Three Oaks Hospice is seeking a compassionate Registered Nurse to deliver skilled nursing care to patients in their homes and coordinate with the interdisciplinary team. You will support patients and families, ensuring timely care planning and adherence to palliative goals.

The role requires an accredited nursing degree, an active RN license, CPR certification, and strong communication as well as organizational skills. Hospice experience is preferred, with on-call responsibilities included.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health preferred.
• Current CPR certification.
• Proficient skill using a computer; Microsoft Office experience preferred.
• Strong interpersonal, communication, and organizational skills.
• Demonstrate ethics and compliance in nursing practice.

Responsibilities

• Perform initial and ongoing assessments of patients and determine needs.
• Coordinate care with the interdisciplinary team and family.
• Educate patients/families about disease process, self-care, end-of-life care, nutrition, and care plans.
• Initiate preventive and rehabilitative nursing procedures.
• Document progress notes showing goals and outcomes.
• Coordinate services and prioritize needs with the care team.
• Communicate patient needs and intervention outcomes to physicians and staff.
• Assess caregiver ability and adapt care plans as needed.
