Job summary

Three Oaks Hospice, Inc. is hiring registered nurses to deliver compassionate hospice care in patients' homes. You will assess patients, implement and adjust care plans with the interdisciplinary team, and educate families about end-of-life needs.

The role requires an active RN license, CPR certification, and strong communication and coordination skills to support patients, families, and caregivers in challenging times.

Qualifications

• Graduate of an accredited Associate or bachelors degree in Nursing.
• Current state license as an RN and driver's license.
• Two years experience as an RN in a clinical care setting; hospice or home health preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.

Responsibilities

• Initial and ongoing comprehensive assessments of the impact of terminal disease on patient needs and ADLs.
• Implement the individualized POC and revise as needed with the care team.
• Educate patient and family about disease, self-care, end-of-life care and nutrition.
• Provide nursing training to other staff as needed.
• Prepare clinical/progress notes showing progress toward goals.
