Job summary

Three Oaks Hospice, Inc. is seeking a qualified RN to administer skilled nursing care to patients in their homes, coordinating with the interdisciplinary team to optimize comfort and quality of life.

The role involves direct patient care, education for families, and thorough documentation of progress. Qualified candidates will hold an RN license, CPR certification, and a couple of years of clinical RN experience; hospice or home health experience is preferred.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ RN experience in a clinical setting; hospice/home health preferred.
• Current CPR certification; proficient with computer use and Office.
• Strong interpersonal, communication, and organizational skills.

Responsibilities

• Administer skilled nursing care for clients of all ages in their home or living setting.
• Coordinate care with the interdisciplinary team and referring agency.
• Perform initial and ongoing assessments of patient needs and ADLs.
• Implement the plan of care, provide education to patient/family, and document progress.
• Provide on-call responsibilities and respond to patient/family needs as assigned.
• Communicate changes in patient status to the physician and IDT.
