Job summary

Three Oaks Hospice, Inc. is seeking a compassionate Registered Nurse to provide skilled nursing care to patients in their homes. You will coordinate with an interdisciplinary team, families, and referring agencies to ensure high-quality end-of-life care.

The role requires an active RN license, CPR certification, and prior hospice or home health experience. You will participate in care planning, on-call coverage, and documentation while delivering patient- and family-centered care.

Qualifications

• Must be a graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and driver’s license.
• Two years’ RN experience in a clinical setting; hospice/home health preferred.
• Current CPR certification.
• Proficient with computers; Microsoft Office experience preferred.
• Strong interpersonal, communication, and organizational skills; high ethics.

Responsibilities

• Administer skilled nursing care to clients of all ages in their homes.
• Coordinate care with the interdisciplinary team and families.
• Perform comprehensive assessments of patient needs and ADLs.
• Develop and revise the plan of care with input from the IDT.
• Provide training and support to staff as needed.
• On-call responsibilities and after-hours coverage.
• Maintain documentation and progress notes for ongoing care.
• Communicate changes in patient needs to physicians and team.
