Job summary

Three Oaks Hospice, Inc. is seeking a licensed RN to provide skilled nursing care in patients’ homes, coordinating with the interdisciplinary team, families, and referring agencies to ensure compassionate end-of-life care.

You will perform comprehensive assessments, develop and revise care plans, educate families, document progress, and participate in on-call rotations while upholding ethical and clinical excellence.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and valid driver's license.
• Two years RN experience; hospice or home health preferred.

Responsibilities

• Assess impact of terminal disease on patients’ needs and ADLs.
• Implement and revise the plan of care (POC) as needed.
• Educate patient and family about disease, self-care, and end-of-life care.
• Provide training to other staff as needed.
• Coordinate all patient and family services with the IDT.
• Use case management approach and refer to other services as needed.
• Communicate changes in patient needs to physicians and team.
