Job summary

Three Oaks Hospice is seeking a compassionate Registered Nurse to administer skilled nursing care in patients' homes or residences. You will coordinate with the interdisciplinary team and families to ensure high-quality end-of-life care and comfort. Active RN license and CPR required, with hospice/home health experience preferred.

The role emphasizes coordinated care, on-call responsibilities, and maintaining meticulous clinical documentation within a supportive, patient-centered environment.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient use of computers; Microsoft Office experience preferred.

Responsibilities

• Initial and ongoing comprehensive assessments of patients' physical, functional, psychosocial, and environmental needs.
• Coordinate care with the interdisciplinary team, patient, family, and referring agency.
• Educate patient and family about disease process, self-care, end-of-life care, nutrition and dietary needs; provide staff training.
• Initiate preventive and rehabilitative nursing procedures as needed.
• Prepare clinical and progress notes showing progress toward goals.
• Coordinate all patient and family services with IDT and prioritize needs.
