Job summary

Three Oaks Hospice, Inc. is seeking a dedicated Registered Nurse to administer skilled nursing care for clients in their homes, coordinating care with the interdisciplinary team to ensure high-quality hospice services.

The role emphasizes compassionate, patient-centered care across ages and settings. The ideal candidate will hold an RN license, CPR certification, and at least two years of clinical RN experience, with hospice or home health experience preferred.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred
• Current CPR certification
• Proficient skill using a computer is required; Microsoft office suite experience is preferred
• Strong interpersonal skills and ability to provide effective support to the patient and family
• Demonstrate skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles
• Strong communication and organizational skills and high-standard of ethics and compliance

Responsibilities

• Initial and ongoing comprehensive assessments of the impact of terminal disease on the patient’s needs.
• Implement the individualized plan of care and revise as necessary.
• Educate patient and family about disease process, self-care, end-of-life care, nutrition and dietary needs.
• Provide training to other staff as needed.
• Coordinate all patient/family services and prioritize needs with the IDT.
• Inform physician and other personnel of changes in patient needs and outcomes.
