Job summary

Three Oaks Hospice is seeking skilled RNs to administer nursing care for hospice patients in their homes and coordinate with an interdisciplinary team. The role emphasizes compassionate patient/family support and adherence to end-of-life care standards.

Responsibilities include assessments, care planning, education, and on-call coverage. A valid RN license and CPR certification are required, with hospice/home health experience preferred.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and valid driver’s license.
• Two years’ RN experience in a clinical care setting; hospice/home health preferred.
• Current CPR certification.
• Proficient computer skills; MS Office experience preferred.
• Strong interpersonal, communication, and organizational abilities.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patients’ needs and ADLs.
• Implement the individualized care plan and revise as needed.
• Educate patients and families about disease, self-care, end-of-life care.
• Coordinate care with the interdisciplinary team and referral sources.
• Provide on-call nursing services as assigned.
• Document progress notes and maintain care records.
