Chapters Health System, Inc. seeks a Per Diem RN to assess and support patients and families in hospice care.

The role coordinates with the Interdisciplinary Group (IDG) and initiates the Plan of Care, delivering compassionate palliative care across assigned facilities and home visits. The position requires RN licensure, BLS, EMR experience, and a flexible, English‑proficient communicator capable of handling the emotional demands of terminal care.

Qualifications

• Current RN license in the state of employment.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• Minimum of 1 year nursing experience in hospice or hospital settings preferred.
• Experience using EMR/EHR systems.
• Valid driver’s license and automobile insurance per company policy.
• Able to communicate effectively in English (verbal and written).
• Flexible schedule to meet patient needs in Hospice House.
• Ability to perform essential physical tasks including lifting patients/equipment.

Responsibilities

• Assess patient/family needs and coordinate the Plan of Care with the Interdisciplinary Group (IDG).
• Provide palliative and supportive care to patients and families.
• Complete clinical admissions for services and obtain physician orders.
• Initiate Plan of Care and communicate with physicians, staff, patients and families.
• Travel to assigned facilities and home visits as required; adhere to scheduling and safety protocols.
