Job summary

Chapters Health System is seeking a Per Diem RN to assess patient and family needs, coordinate the Plan of Care with the IDG, and provide palliative and supportive care to patients and families.

Responsibilities include admission coordination, education, and communication with physicians and staff. The role requires active RN license, BLS, hospice or hospital experience, EMR familiarity, and travel as needed.

Qualifications

• Current RN license in the state where the employee will be working.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• Minimum one year nursing experience in hospice or hospital preferred.
• Experience with EMR/EHR systems.
• Valid driver's license and transportation to visit schedules.
• Ability to manage emotional stress of end-of-life care.
• Ability to perform essential functions including lifting and mobility.
• Must be able to communicate in English (verbal and written).

Responsibilities

• Provides and manages direct care to patients and families with IDG involvement.
• Coordinates Plan of Care reflecting patient’s evolving needs.
• Educates patients and families about disease, palliative care, and safety.
• Communicates changes in patient condition to IDG and physicians.
• Presents concise reports to IDG; supports timely referrals and discharge.
• Supervises nursing staff and contract nurses as assigned.
• Performs duties in emergencies and adheres to policy guidelines.
