Job summary

Chapters Health System is seeking a Per Diem RN to deliver compassionate palliative and supportive care within the Hospice House and hospital environments. You will assess patient needs, apply the nursing process, and coordinate care with the Interdisciplinary Group to optimize comfort and outcomes.

Qualified candidates hold a current RN license in the state, active BLS, and at least 1 year of hospice or hospital experience; EMR/EHR familiarity and a valid drivers license with reliable

Qualifications

• Current RN license in the state of employment.
• Active BLS for healthcare professionals (AHA or Red Cross).
• Minimum one (1) year nursing experience in hospice or hospital setting preferred.
• Previous EMR/EHR experience is preferred.

Responsibilities

• Provides direct care to patients and families as part of the Interdisciplinary Group (IDG).
• Coordinates the Plan of Care and ensures it reflects evolving patient needs.
• Educates patient and family about disease, palliative interventions, care giving and safety.
• Completes documentation and obtains physician orders; facilitates admissions and discharges.
• Performs additional duties as assigned and supports bereavement interventions with psychosocial staff.
