Job summary

Chapters Health System is seeking a Per Diem RN to provide compassionate, patient- and family-centered care within the Hospice House setting. The RN will assess, plan, and deliver palliative care in collaboration with the Interdisciplinary Group (IDG), ensuring treatment aligns with patient goals and comfort needs.

The role requires current RN licensure, BLS certification, and experience in hospice or hospital settings.

Qualifications

• Current RN license in the state of employment.
• Active BLS for healthcare professionals (AHA or Red Cross).
• Minimum one year hospice or hospital experience preferred.
• Experience with an EMR/EHR system.

Responsibilities

• Provide direct care to patients and families as part of Interdisciplinary Group (IDG).
• Coordinate the Plan of Care with IDG.
• Educate patient and family about disease process and comfort measures.
• Report changes in patient condition to IDG.
• Supervise nursing staff as needed and participate in emergencies.
