Job summary

Affinity Health Management is seeking a registered nurse to plan, organize, and direct hospice care in home and community settings. The role emphasizes patient-centered assessment, care planning, and family education to improve comfort and quality of life.

The nurse will perform comprehensive assessments, coordinate care with the interdisciplinary team, manage medications, and provide guidance to patients and caregivers while ensuring compliance with regulations and safety standards.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with a license in good standing to practice.
• Minimum of 1 year of professional nursing experience in last 3 years; public health/home care/hospice preferred.
• Management experience not required; responsible for supervising hospice aides.
• References and criminal background checks required by regulation.
• Excellent observation, verbal and written communication skills, problem solving, basic math skills.
• Prolonged walking/standing; able to lift, position and transfer patients; visual/hearing acuity.
• Must have a valid driver’s license and insured vehicle.
• Possesses CPR certification (hands-on demonstration required).

Responsibilities

• Completes initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Writes and initiates plan of care and revises as patient status changes.
• Initiates appropriate preventive and rehabilitative nursing procedures and administers medications as prescribed.
• Counsels patient and family on nursing needs and self-care.
• Acts as Case Manager when assigned and coordinates patient care for caseload.
• Maintains accurate clinical notes and reports changes in patient condition.
• Works with the interdisciplinary team and participates in meetings.
