Job summary

Three Oaks Hospice in Colorado is hiring a full-time Registered Nurse to administer skilled nursing care for patients in their homes across Castle Rock, Parker, Centennial, Highlands Ranch. The role coordinates care with the interdisciplinary team and families and assumes responsibility for care coordination.

Responsibilities include comprehensive assessments, implementing the plan of care, patient and family education on end-of-life care, case management, and on-call coverage.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and valid driver's license.
• Minimum two years’ RN experience in a clinical setting; hospice/home health preferred.
• Current CPR certification and comfort with computer use.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patients in their homes.
• Coordinate care with the interdisciplinary team and referring agencies.
• Educate patients and families about disease processes, end-of-life care, and self-care.
• Initiate appropriate nursing procedures and document progress notes.
• Coordinate services and prioritize patient/family needs with the IDT.
• Provide on-call coverage as assigned.
• Adapt care plans based on acuity and patient needs.
