Job summary

Three Oaks Hospice in Commerce City area seeks a compassionate Registered Nurse to administer skilled nursing care at patients' homes, coordinate with the interdisciplinary team, and manage care coordination. Responsibilities include assessments, implementing care plans, educating patients and families on end‑of‑life care, training staff, documenting progress, and on‑call coverage.

Strong communication, computer proficiency and a current CPR certification are required; a valid driver's license

Qualifications

• Graduate degree in Nursing (ASN or BSN) and valid state RN license.
• Two years of RN experience in a clinical setting; hospice or home health preferred.
• Current CPR certification and strong communication skills.
• Proficient computer skills, including Microsoft Office.

Responsibilities

• Conduct initial and ongoing comprehensive assessments of patient needs, including physical, functional, psychosocial, and environmental factors.
• Implement the individualized care plan and update as needed.
• Educate patients and families on disease process, end-of-life care, and self-care techniques.
• Provide training to other staff as needed.
• Coordinate all patient/family services with the interdisciplinary team.
• Document progress notes showing advancement toward goals.
• Perform on-call duties as assigned.
