Job summary

Three Oaks Hospice is seeking a dedicated RN to deliver skilled nursing care in patients’ homes within the North Territory of our Colorado network. You will coordinate care with the interdisciplinary team, patients, families, and referring agencies, ensuring compassionate, compliant, and on-time delivery of services.

Responsibilities include comprehensive assessments, implementing the plan of care, educating families, on-call coverage, and collaboration with physicians to tailor care.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.
• Strong interpersonal, communication, and organizational skills.
• Ability to cope with family emotional stress and diverse lifestyles.
• Commitment to ethics and compliance in nursing practice.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patients.
• Implement the individualized plan of care and revise as needed.
• Educate patients and families on disease process, end-of-life care, nutrition, and self-care.
• Provide on-call nursing services to patients/families as assigned.
• Coordinate care with the interdisciplinary team and referring agencies.
• Document progress notes showing adherence to goals.
