Job summary

Elevation Hospice of Colorado is hiring a Registered Nurse to provide skilled nursing care for patients in their home setting within the Denver area, with a Monday–Friday schedule of four 10-hour shifts. You will coordinate care with the interdisciplinary team, conduct initial and ongoing assessments, implement the plan of care, educate patients and families, and support comfort at end of life.

Qualifications

• Graduate of an accredited RN program with licensure.
• Strong clinical skills in assessing patients and planning care.
• Excellent communication with patients, families, and the care team.
• CPR certification and valid driver’s license required.

Responsibilities

• Assess patients' physical, psychosocial, and environmental needs.
• Coordinate care with the IDT and referring agencies.
• Implement and revise the plan of care; document progress.
• Educate patients and families on disease, care options, and comfort.
• Provide on-call support and ensure timely care delivery.
• Maintain accurate clinical notes and comply with policies.
