Job summary

Three Oaks Hospice in Colorado is seeking a compassionate Registered Nurse to administer skilled nursing care for patients in their homes, coordinating with the interdisciplinary team and families. The role focuses on end-of-life care, assessments, and care plan execution.

Responsibilities include comprehensive assessments, care coordination, patient/family education, and providing on-call support. A nursing degree, active RN license, CPR, and hospice/home health experience are preferred for

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing
• Current state licensed RN and valid driver’s license
• Two years’ RN experience in a clinical setting; hospice/home health preferred
• Current CPR certification
• Proficient with computers; MS Office experience preferred
• Strong interpersonal and communication skills; ethical and compliant practice

Responsibilities

• Perform initial and ongoing comprehensive assessments of patients' needs, risk for grief, and ADLs
• Implement individualized plan of care and revisions as needed
• Educate patients and families on disease process, end-of-life care, nutrition, and self-care
• Initiate preventive and rehabilitative nursing procedures
• Document progress toward goals and coordinate care with the interdisciplinary team
• Coordinate patient/family services and prioritize needs with the IDT
• Communicate changes in patient needs to physicians and team
• Determine scope and frequency of services based on acuity and family needs
• Manage on-call responsibilities and provide on-call care as assigned
• Other duties as assigned
