Three Oaks Hospice is seeking a dedicated RN to administer skilled nursing care for patients in their homes or in residence, coordinating care with the interdisciplinary team and referring agencies. The role requires an active RN license and Colorado residency; hospice/home health experience is preferred.

You will perform comprehensive assessments, tailor the plan of care, educate patients and families, and provide on-call support.

Qualifications

• Graduate of an accredited Associate or bachelor's degree in Nursing.
• Current state RN license and driver's license.
• Two years' RN experience in clinical care; hospice/home health preferred.
• Current CPR certification; proficient computer skills; MS Office preferred.
• Strong interpersonal, communication, and organizational skills.

Responsabilités

• Administer skilled nursing care for clients in residence.
• Coordinate care with the interdisciplinary team and referring agencies.
• Perform initial and ongoing assessments of patient needs and ADLs.
• Develop and revise the plan of care; provide patient/family education.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare progress notes demonstrating goal advancement.
• Coordinate services with IDT and manage on-call responsibilities.
• Communicate changes in patient needs to physicians and team.
