Job summary

Three Oaks Hospice is seeking an On-Call Registered Nurse (RN) to join our team, delivering skilled nursing care to patients in their homes within the Tarrant and Parker County area. You will coordinate with the interdisciplinary team, patients, families, and referring agencies to ensure seamless care.

The role requires an RN license, CPR certification, and at least two years of clinical RN experience, with hospice or home health experience preferred.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred
• Current CPR certification
• Proficient using a computer; Microsoft Office experience preferred
• Strong interpersonal skills and ability to provide effective support to the patient and family
• Demonstrate skill in nursing practice and tolerance of family emotional stress

Responsibilities

• Administer skilled nursing care for clients in their place of residence.
• Coordinate care with the Interdisciplinary Team, patient/family and referring agency.
• Coordinate of care across the care continuum.
