Job summary

Three Oaks Hospice, Inc. is seeking an experienced Registered Nurse to provide skilled nursing care to patients in their homes within Commerce City, Denver and surrounding areas. You will coordinate with the interdisciplinary team, educate families, and ensure compassionate end-of-life care.

The role requires an RN license, a nursing degree, CPR certification, and at least two years of clinical RN experience (hospice/home health preferred). Excellent communication and ethics are essential.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice/home health preferred.
• Current CPR certification.
• Proficient with computers; MS Office experience preferred.
• Strong interpersonal and communication skills; high ethics and compliance.

Responsibilities

• Perform initial and ongoing assessments of patients and families.
• Coordinate care with the interdisciplinary team and referring agencies.
• Educate patients and families about disease, end-of-life care, and self-care.
• Provide on-call coverage as assigned.
• Document progress notes and plan of care; adjust as needed.
• Coordinate services and resources across the care team.
• Assess caregiver ability to meet patient needs throughout care.
