Zusammenfassung

Gentiva is seeking a compassionate Hospice RN / Case Manager to lead patient care in home settings across North Carolina. You will assess needs, coordinate an interdisciplinary team, and implement Plan of Care with a focus on comfort and dignity.

Responsibilities include in-person and telephonic assessments, symptom management, caregiver education, and meticulous documentation to meet regulatory standards. A current RN license and CPR are required.

Qualifikationen

• Registered Nurse in good standing with a current license.
• Minimum of 1 year RN experience in hospice, palliative care, or home health.
• Hospice or palliative care experience preferred.

Aufgaben

• Perform comprehensive patient assessments and reassessments in home or alternate settings.
• Deliver nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care with visit frequency and risk identification.
• Coordinate care with physicians, LPNs, CNAs, social workers and chaplains.
• Educate patients and caregivers on disease, end-of-life care, medications, nutrition, and self-care.
• Communicate changes in condition to the care team and document timely records.
• Support continuity of care across home health, palliative, and hospice transitions.
• Maintain regulatory-compliant documentation and participate in quality initiatives.
