Job summary

Gentiva is seeking a Registered Nurse (RN) Hospice for a nursing role in Washington, North Carolina. You will lead with heart, delivering personalized hospice and palliative care in patients’ homes or other living environments.

Responsibilities include guiding families, coordinating with physicians, and ensuring high-quality care in a compassionate setting. The position requires RN licensure, at least 1 year of RN experience in hospice or related fields, and a commitment to comprehensive patient

Qualifications

• RN required.
• Minimum 1 year RN experience in acute care, oncology, long-term care, hospice or home health.

Responsibilities

• Be the primary contact for patients and families, guiding them through the hospice journey.
• Deliver and document skilled nursing care per Plan of Care in home, assisted living, or patients' homes.
• Perform initial and ongoing assessments and adjust care as needed.
• Collaborate with an interdisciplinary team to build personalized care plans.
• Provide emotional, physical, and spiritual support to patients and loved ones.
