Gentiva is seeking a compassionate Hospice RN / RN Case Manager to lead patient care in home and community settings. You will perform comprehensive assessments, implement and document Plan of Care, and coordinate with physicians, LPNs, CNAs and social workers to ensure high-quality end-of-life care with empathy.

You will educate families, support emotional and spiritual needs, and participate in on-call activities while maintaining accurate records and complying with regulatory standards.

Formación

• Active RN license in the state of employment or eligibility.
• Minimum 1 year RN experience in acute care, hospice or home health; hospice/palliative care preferred.
• CPR certification and valid driver's license required.

Responsabilidades

• Lead as primary point of contact for patients and families.
• Deliver and document skilled nursing care per Plan of Care at patients' homes or equivalent locations.
• Perform initial and ongoing assessments; adjust care as needed.
• Coordinate with IDG to develop/refine personalized care plans.
• Provide education and bereavement support to patients and families.
