Job summary

Gentiva Hospice is seeking a compassionate Hospice RN/Case Manager in North Carolina. You will lead direct nursing care in patients’ homes or alternative settings, coordinating with an interdisciplinary team to develop and adjust individualized care plans.

Strong assessment, education, and communication skills are required. You will partner with patients and families to ensure comfort, dignity, and quality of life at end of life, while meeting regulatory standards and organization guidelines.

Qualifications

• Current license to practice nursing in the state of employment
• CPR Certification
• Current automobile insurance and valid driver’s license
• Hospice or palliative care experience preferred

Responsibilities

• Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments
• Deliver nursing interventions and symptom management aligned with goals of care
• Develop and update plans of care including visit frequency and risk identification
• Coordinate care across interdisciplinary teams including physicians
• Educate patients and caregivers about disease processes, self-care, and end-of-life care
• Communicate changes in condition to the care team for timely interventions
• Support continuity of care and transitions in levels of care
• Document timely clinical records and maintain quality documentation
• Maintain relationships with IDG and involve team when care plans require collaboration
• Participate in on-call and patient support activities as applicable
