Gentiva Hospice is hiring an RN Case Manager to lead patient assessments, coordinate care, and deliver hands-on nursing in patients' homes. You will work with an interdisciplinary team to ensure compassionate, personalized end‑of‑life care and support for patients and families.

Responsibilities include education, care planning, documentation, and on‑call participation, with emphasis on regulatory compliance and patient advocacy in a home‑based setting.

Qualifikationen

• Current RN license in the state of employment.
• CPR certification is required.
• Must maintain automobile insurance and a valid driver’s license.

Aufgaben

• Perform comprehensive patient assessments and reassessments, including in‑person and telephonic visits.
• Deliver skilled nursing care and symptom management per Plan of Care in patients' homes or other settings.
• Develop and update individualized care plans and determine visit frequency.
• Coordinate care with interdisciplinary team and consult physicians as needed.
• Educate patients and caregivers on disease, care techniques, and end‑of‑life processes.
