Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to lead patient assessments, coordinate interdisciplinary care, and guide families through the hospice journey in patients' homes or preferred settings. Strong nursing skills and compassionate communication are essential.

The role requires collaboration with physicians, LPNs, CNAs, social workers, and chaplains to develop personalized care plans and ensure timely, quality care. A current RN license and CPR are required.

Qualifications

• RN with current license to practice in the state of employment.
• Minimum one year of RN experience, hospice or home health preferred.
• CPR certification required or as per policy.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Deliver and document skilled nursing interventions and symptom management.
• Develop and update individualized care plans and visit frequencies.
• Coordinate care with physicians, social workers and other IDG members.
• Educate patients and caregivers on disease, end-of-life care and medications.
