Job summary

Responsibilities include education for families, coordinating care across disciplines, and maintaining accurate documentation while adhering to regulatory standards. This role is based in diverse home settings with patient-centered support.

Qualifications

• RN with at least 1 year experience in hospice, palliative care, or home health.
• Strong communication and empathy for patients and families.
• Ability to develop and adjust individualized Plans of Care and coordinate with an IDG.

Responsibilities

• Perform comprehensive patient assessments and reassessments in person or by telecommunication, with education for families and caregivers.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care, including visit frequency and risk identification.
• Coordinate care across interdisciplinary teams, including physicians and nurses.
• Educate patients and caregivers about disease, self-care, end-of-life care, medications, and nutrition.
• Communicate changes in condition to the care team and adjust care as needed.
• Document timely clinical records and maintain infection control and compliance.
• Support continuity of care and transitions across levels of care; participate in on-call activities.
