Job summary

Gentiva Hospice in Tennessee seeks an experienced RN Case Manager to lead patients and families through the hospice journey. You will provide skilled nursing care at home, perform assessments, and collaborate with physicians, LPNs, CNAs, social workers, chaplains, and other IDG members to tailor care plans.

You will educate caregivers, support bereavement needs, document care, and ensure compliance with regulatory standards while participating in on-call duties as required.

Qualifications

• Current license to practice nursing in the state of employment.
• CPR Certification.
• Current automobile insurance and valid driver’s license.
• One year RN experience in acute care, oncology, long-term care, hospice or home health preferred.

Responsibilities

• Be the primary point of contact for patients and families, guiding them through the hospice journey.
• Deliver and document skilled nursing care based on Plan of Care in home or alternative settings.
• Perform initial and ongoing assessments and adjust care as needed.
• Coordinate care with an interdisciplinary team to refine personalized care plans.
• Provide education and bereavement support to patients and families.
• Maintain timely clinical records and ensure infection control compliance.
• Participate in on-call and patient support activities as applicable.
