Job summary

Gentiva Hospice is seeking a dedicated RN Case Manager to lead compassionate, in-home hospice care for patients and families in Tennessee. You will assess needs, develop plans of care, and coordinate an interdisciplinary team to support comfort and dignity at end of life.

Responsibilities include delivering skilled nursing care, documenting assessments, educating families, and ensuring timely communication and care plan updates.

Qualifications

• Minimum 1 year RN experience in hospice, home health, or equivalent.
• Hospice or palliative care experience preferred.
• Current RN license in state of employment.

Responsibilities

• Perform comprehensive patient assessments and reassessments and educate patients, family members and caregivers as appropriate.
• Deliver nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including visit frequency to meet individual needs.
• Coordinate care across interdisciplinary teams including physicians and nurse practitioners.
• Provide education on disease process, self-care techniques, end-of-life care, medications.
• Document timely clinical records and maintain infection control compliance.
• Participate in on-call services as applicable.
